Crankshaft Issue - Shifted Forward

Good Evening,

Noticed some light, occasional tapping on startups over the weekend. When checking for the noise, didn’t notice anything out of place. After some more driving, got a loud rattle and then heard something fall from truck (went back and couldn’t find anything).

looks like what I heard was the crank pulley bolt falling from the truck and also (after inspection) it looks like the light tapping was the fan hitting the crank pulley. Truck runs and starts ok with belt squealing on startup and with no fan contact now.

Attached are some pics from underneath. Just looking for some insight on how bad this is and maybe some advice on how to proceed.

Background: 1994 NAS 90 with a 4.6. 4.6 GEMS from AB replaced the 3.9. Work was done about 12 years ago.

when you fix it you need a spacer between the bolt and pulley to keep it from fretting. The pulley can move quite a bit without it

TWS sells one, or you can turn down an old timing gear

Thank you for the help. Wasn’t sure if I was in a major engine situation or not with this.

Will get another bolt and spacer on order.
